What is the Service?

• You will live in a home with other young men.
• On your first day in the home, you will be introduced to your treatment team, to the other residents and to the daily routine of the home.
• You will meet privately with a professionally credentialed clinician who will listen to your concerns, help you identify areas in your life that you would like to change and help you develop a plan to make these changes.
• You will be supported by the clinician and the treatment team as you take steps to bring about these changes.
• You will receive individual, family and group counseling.
• You will meet with the psychiatrist who will complete an evaluation and may prescribe medication for you, if needed.
• You will be supported by staff to maintain your connections with family and other significant people in your life.
• You will be expected to attend school, prepare for work, or be involved in a vocational training program. You will make choices regarding which option is right for you. Staff will assist you in accessing an educational or vocational program.
• You may also choose to obtain a job and work in the community.
• You will participate in activities to help you develop independent living skills such as food shopping, opening a bank account and completing household chores.
• You will earn weekly allowance through a behavioral modification point system.
• You will receive assistance in obtaining medical, dental, and other health related services.
• You can take advantage of the on-site basketball court and the program membership at the local community center.
• You will help schedule and participate in group recreational outings.
• You can earn independence time which will allow you to socialize independently in the community.
• Your spirituality and religious preferences will be supported, and you will be given every opportunity to practice and participate in the religion of your choice.
• You will be assisted in finding a living situation that best meets your needs when you are ready to leave the home.
• You also will be assisted in arranging any other supports and services you will need to be successful upon leaving the program.
