As a leading health and human services provider in Philadelphia, PATH knows that the social determinants of health affect a wide range of health and quality-of-life outcomes for its participants and their families. With PATH as the anchor, PATH@1919 will provide key resources and services to the Northeast community.
How does it work?
PATH partners use shared space at our 1919 Cottman Avenue headquarters on a regular schedule. Some partners have “satellite” offices on-site, and some use our Community Room, Training Room, or other spaces for specific events. Organizations that are not official community hub partners can also use our space for community events. PATH and partner staff are trained to better understand each other’s current services, to help us make the most helpful connections possible based on each individual’s needs.
