Why is the service offered?

The Continuity of Care Program helps you to identify the services and supports that will best meet your needs.

What is the service?

  • When you first contact PATH, you will speak with a clinician who will listen to your concerns and schedule an initial intake assessment with you.
  • At the intake appointment, you will meet privately with a professionally credentialed clinician to complete a comprehensive assessment which usually takes about 2 hours.
  • The clinician will ask you questions about your life and help you identify those areas you would like to change.
  • You and the clinician will work together to identify your strengths and challenges and define the services at PATH that will best meet your needs.
  • The clinician will arrange for you to receive those services and see a PATH psychiatrist, if indicated.
  • When appropriate, the clinician will assist you in obtaining information and referrals to other specialized treatment facilities.
