Resource Coordinator - Specialized

Job Summary
Assist Young Adults between the ages of 18 and 25 in gaining access to needed psychiatric, medical, social, residential, education, financial and other services appropriate for the individual. Attend meetings at the Office of Behavioral Health to give input and analyze the needs of this unique population which is transitioning from the child/adolescent service system to the adult service system. Make recommendations to the Office of Behavioral Health regarding systemic issues and resource development. Coordination and monitoring of individual’s treatment/rehabilitation services.
Specific Duties

Attend meetings at OMH/MR and OVR as assigned to define service gaps and make recommendations regarding resources needed to develop a comprehensive service delivery system for the targeted population.

Complete all RC State mandated paperwork, i.e., Service Documentation Forms, Outcome Reports, Psychosocial Assessments, Strength Assessments, etc.

Complete individual Personal Goal Plans with emphasis on individual’s strengths and needs.

Complete all other documentation required by regulatory agencies, funding sources and internal procedures.

Assess and evaluate new RC individuals when assigned to caseload.

Be aware of RC individual’s financial liabilities, coordinate any financial changes with the Billing Department; and assist individuals in securing third party resources when indicated.

Attend RC Team Meetings at PATH, internal task forces and all appropriate RC meetings outside of agency, i.e., TCM All Systems meetings.

Linking individual with services by assisting in locating and obtaining appropriate services for individuals.

Monitoring service delivery by maintaining regular contact with individual and service providers/programs involved with individual.

Gaining access to services with aggressive and creative attempts to help individuals gain resources and services which are needed. This includes a strong role as individual advocate.

Informal support and network building to help individual increase his/her support system outside of the mental health system.

Outreach including routine home visits for RC individuals.
Position Requirements
Preference for one year experience in the mental health field. Good written and verbal communication skills. Ability to analyze system needs. (Valid driver’s license; ability to meet agency insurance requirements; use of personal auto preferred.)

$27,498 per Year - $13.22 per Hour
