Psychosocial Rehabilitation Specialist

Job Summary

To utilize a variety of approaches and interventions to promote recovery within a site based program as well as provide successful community integration experiences. Provide individualized support and skill teaching in the community to promote new role development. Assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual.

Specific Duties

  • Attend trainings on recovery; psychiatric rehabilitation, trauma, supported employment practices and cultural competency. Keep apprised of new research and developments in these areas through journals, web sites and literature.
  • Co-facilitate community meetings to nurture a peer support culture.
  • Assist in the development and implementation of any other education and support groups deemed beneficial by the Stakeholder Committee.
  • Assist individual in developing and practicing the skills defined on their psychiatric rehabilitation plans. This may include accompanying individuals to appointments, leisure activities or other activities of interest including religious, educational or vocational to provide support and encouragement.
  • Link individuals to community resources and peer supports to promote community integration.
  • Actively engage individuals in services including outreach visits at the individual's home, crisis center or outpatient facility.
  • Carry individual caseload under supervision of Program Manager.
  • Responsible for developing treatment plans and Annual Psychosocial Evaluations.
  • Ensure all required documentation is completed in a timely, comprehensive and legible manner.
  • Collaborate with involved personnel in the community such as GED instructors, college professions, faith-based representatives, volunteer and employment supervisors as assigned by Program Manager.
  • Consult and collaborate with all team members including psychiatrist on a regular basis.
  • Attend trainings on relevant topics such as recovery, substance abuse and psychiatric rehabilitation.
  • Be aware of and follow PATH policies and procedures including those regarding confidentiality.
  • Participate in quality improvement and data gathering as assigned.
  • Drive PATH vehicle for program reasons.
  • Be aware of financial liabilities, coordinate any financial changes with the billing department and assist in securing their party resources when indicated.
  • Assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual.
  • Provide 24 hour/7 days a week emergency beeper coverage on a rotating basis.

Position Requirements

Experience working with individuals who have a mental illness and/or substance abuse problem. Must receive Certified Psychiatric Rehabilitation Practitioner (CPRP) Credential by the Fall 2008 credentialing cycle which is determined by the United States Psychiatric Rehabilitation Association (USPRA). Experience with Boston University Model of Psychiatric Rehabilitation preferred. Valid PA Driver's license.
