Program Supervisor III ADTP Program

JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by managing an effective team to provide quality services.  Responsible for management and supervision of PATH’s Adult Day and Older Adult Services, compliance with licensing requirements, and the promotion of PATH’s mission and principles.

Specific Duties

  1. Supervise and manage the day to day operations of the Adult Day Program and Older Adult Services.
  2. Ensure proper staff coverage of individuals enrolled in the program and provide coverage as needed.
  3. Keep the Program Director informed of all program related issues.
  4. Chair all supervisory meetings.
  5. Act as activities coordinator and sign off on activities scheduled for all group rooms.
  6. Maintain and enforce compliance with all funding and/or licensing entities, such as but not limited to the Office of Developmental Programs’ Adult Day Training Regulations; 55 PA. Code Chapter 2380, Older Adult Daily Living Center Regulations Philadelphia Mental Retardation Services, and the Department of Labor’s Wage and Hour Division.
  7. Assist with preparation for annual State Inspection for Licensing and ensure minimal citations
  8. Plan for new and improved consumer service in accordance with best practices.
  9. Market the ADP and OAS to the community and obtain referrals.
  10. Review and evaluate prospective Adult Day and Older Adult individuals; schedule meetings with prospective new individuals, facilitate completion of necessary documentation; facilitate smooth transition and complete agency admission documentation.
  11. Complete admission and discharge planning for individuals entering or leaving the program and complete all necessary documentation on a timely basis.
  12. Oversee the scheduling of ISP meetings, the identification and implementation of goals and the completion of required documentation for all group rooms.
  13. Oversee the completion and accuracy of participant physicals.
  14. Review individuals’ files to ensure accuracy and that all information is complete.
  15. Attend, as needed, individual team meetings, ISP meetings, QEST reviews, Visions for Equality monitoring and Support Coordination monitoring with or in the absence of Adult Day and Older Adult Program Specialists.
  16. Fulfill duties of Program Specialists in their absence.
  17. Oversee the completion of annual reports, quarterly reviews and monthly summaries for caseload as well as attend meetings such as Individual Support Plans and Support Intensity Scale Assessments.
  18. Meet productivity standards of the Adult Day and Older Adult Services.
  19. Oversee the completion of Service Activity Logs.
  20. Review billing of contract agencies.
  21. Work with Director to oversee the management and accuracy of funding units for individuals.
  22. Develop proposals and update paperwork and policies in coordination with the Program Director or Intellectual Disabilities Director, as requested.
  23. Complete annual performance appraisals of assigned staff.
  24. Provide lobby coverage as needed.
  25. Provide instructional coverage with individuals as needed.
  26. Oversee the completion of fire safety training to the Adult Day and Older Adult Services participants on an annual basis.
  27. Coordinate, conduct and document monthly fire drills.  
  28. Manage the program’s petty cash and track individual’s petty case.
  29. Supervise the completion of monthly check-lists related to building safety, housekeeping     and building maintenance to ensure safety and compliance with regulations.
  30. Provide as needed consultation on Adult Day and Older Adult issues.
  31. Meet regularly for supervision with Program Director.
  32. In conjunction with Human Resources, participate in related investigations.
  33. Refer all requests for references to Human Resources.
  34. Carefully review, follow up, correct and process all incident reports generated by Adult Day and Older Adult Services in a timely fashion.
  35. Participate and complete special projects as assigned.

Qualifications

Master’s degree preferred, Bachelor’s degree required in human services from an accredited college or university. At least three years experience, including supervisory, working directly with people with intellectual disabilities. Excellent clinical, interpersonal and writing skills. Thorough knowledge of Intellectual Disabilities Field and Best Practices.
