MR Day Specialist II

Job Summary

Provide input and assistance in day-to-day training and supervision of individuals in the program. Coordinate and develop initial assessments for clients entering the program. Schedule IPP meetings/reviews for assigned cases and plan program activities to meet IPP goals and objectives. Directly assist individuals in all group activities and community outings. Serve as program specialist for assigned individuals and monitor and document individuals progress. Attend regularly scheduled meetings.

Specific Duties

  • Provide assistance and supervision of individuals during group and individualized activities.
  • Assist in coordination of training and activity planning for individuals in the program.
  • Develop initial assessments for assigned individuals.
  • Coordinate, develop and review IPP’s for assigned individuals.
  • Schedule and chair interdisciplinary team meetings for IPP development/review for assigned individuals.
  • Develop a planned program of activities (individual and group) based on the IPP's of each individual.
  • Prepare material necessary to further IPP objectives.
  • Serve as program specialist for assigned caseload and maintain their individual records.
  • Monitor individuals progress and document in the form of data collection, monthly progress notes, and production cards if indicated.
  • Monitor individuals as they are waiting to go home after the program ends each day, as needed.
  • Drive van for programmatic reasons (during program hours) on an as-needed basis.
  • Attend staff meetings and training scheduled.
  • Must receive 24 training hours.

Position Requirements

At least one year of experience working directly with individuals with disabilities, or Associates degree with at least three years of experience working with individuals with disabilities. Valid driver’s license and ability to drive a 15-passenger van preferred. Physical examination certifying no presence of communicable disease, including a TB test no more than 12 months prior to employment and every two years thereafter.

Education Requirements

Bachelor’s degree from an accredited College or University.

$21,977 per Year / $11.27 per Hour
