Intensive Case Management - Support Specialist

Job Description

Assist Targeted Case Management (TCM) staff in working with clients on TCM caseload. Help clients gain access to needed psychiatric, medical, social, residential, financial and other appropriate services for client.

Specific Duties

Accompany TCM staff on home and community visits when needed.
Work with TCM staff in crisis situations regarding clients on TCM caseload.
Assist and accompany clients as needed in locating and obtaining appropriate services (i.e. residential, medical, psychiatric, social and all other services.)
Complete all required paperwork.
Be aware of TCM client’s financial liabilities; coordinate any financial changes with the Billing Department; and assist clients in securing third party resources when indicated.
Attend all appropriate TCM team meetings and clinical case conferences regarding clients on TCM caseload.
Attend all other internal and external meetings and task forces as required.
Focus on client’s strengths and needs rather than weaknesses and assist with service plans as needed.

Position Requirements

Valid driver’s license and use of personal automobile preferred.

Education Requirements

Bachelor’s Degree in Human Services.
