Continuity of Care Specialist II - Per Diem

Job Summary
Participate in the delivery of direct clinical service. To complete comprehensive biopsychosocial evaluations.

Specific Duties

To participate in face to face comprehensive biopsychosocial evaluations.
To gather information during the biopsychosocial evaluation to assist the psychiatrist in his/her assessment.
To assess individual needs and coordinate efforts directed toward meeting those needs.
To maintain adequate clinical records
To complete all appropriate documentation in order to prepare record for daily case conference.
To determine the need, in conjunction with the Continuity of Care Director, for emergency psychiatric consultation.
To participate in staff meetings as appropriate.
To consult with other PATH staff as well as outside agencies.
To maintain regular communication with Continuity of Care Director.
To be aware of individuals financial liabilities, to coordinate any financial changes with the Billing Department, and to assist individuals in securing third party resources when indicated.
Other related duties as assigned.

Position Requirements

Sensitivity to the need of individuals with a serious mental illness and familiarity with completing biopsychosocial evaluations preferred.

Masters Degree from an accredited college/university in the human services profession with some BH experience preferred.
