Children & Youth Intensive Case Management Program

Who is served?
You are served if you are a child or teenager who is experiencing
serious mental health problems that are causing you to have significant
problems in school, at home or in your community.
Why is the service offered?
The Intensive Case Management Program helps you to resolve crises,
build upon your strengths and overcome problems so you can successfully
meet life’s challenges.
What is the service?
- You will meet with a case manager who will partner with you and your family to achieve your goals.
- You will meet with your case manager at your home, at PATH or in the community.
- You will receive help from your case manager to obtain the services you and your family need such as housing, health insurance, and appropriate school placement.
- You will receive help from your case manager to obtain mental health, substance abuse, or other medical treatment.
- You and your family will receive assistance from your case manager to effectively collaborate with your school and assure that your academic needs are met.
- You will be able to contact a case manager for help whenever you experience a crisis, 24 hours a day, 7 days a week.
- You will be supported by your case manager as you resolve problems and begin to develop confidence in your own abilities to meet life’s challenges.
Where is the service located?
- Services are provided in your home, in the community, in your school or wherever assistance and support are needed.
- The Intensive Case Management offices are located at 8220 Castor Avenue, Northeast Philadelphia, PA 19152

When is the service available?
- Monday through Friday - 9:00 AM to 5:00 PM
- Evening appointments are scheduled as needed.
- A case manager is available 24 hours a day/7 days a week for crisis.
You are eligible to receive services if:
- You reside in Philadelphia.
- You have a psychiatric or psychological evaluation within the past 45 days recommending Intensive Case Management Services. The person or agency completing the evaluation and referral must submit the information to the Philadelphia Department of Behavioral Health. We can help you arrange for the evaluation and referral.
- The Philadelphia Department of Behavioral Health has approved this service for you.
