Who is served?


You are served if you are an adult experiencing serious substance abuse problems that result in frequent crisis evaluations and/or use of detoxification services.


Why is this service offered?

The Substance Abuse Case Management Program helps you better manage your substance abuse problems and achieve a more satisfying and rewarding life.


What is this service?

  • You will meet with a case manager who will form a partnership with you to facilitate your recovery from substance abuse.
  • You will meet with your case manager at your home, at PATH or in the community.
  • You and your case manager will identify goals you would like to achieve and develop a plan to achieve your goals.
  • You will receive help from your case manager to obtain substance abuse, mental health or other medical treatment.
  • You will receive help from your case manager to obtain services such as housing, health insurance, financial benefits and other needed resources.
  • You will be able to contact a case manager for help whenever you experience a crisis, 24 hours a day, 7 days a week.
  • You will receive support and encouragement from your case manager as you recover and develop a life-style free from substance dependence.

Where is the service located?
  • Services are provided in the community, in your home or wherever assistance and support is required.
  • The case management offices are located at 8220 Castor Avenue, Northeast Philadelphia, PA 19152.

When is the service available?
  • Monday through Friday - 8 AM to 6 PM
  • A case manager is available 24 hours a day/7days a week for crisis

You are eligible to receive services if:
  • You reside in Philadelphia.
  • You have had two or more episodes of acute substance abuse treatment during the past year. The person or agency making the referral should contact PATH.
