Resource Coordination Program

Who is served?


You are served if you are an adult experiencing serious mental health problems or you are experiencing both serious mental health and substance abuse problems.


Why is this service offered?

The Resource Coordination Program helps you manage your illness, become involved in your community and achieve a more satisfying and rewarding life.


What is the service?

  • You will meet with a resource coordinator who will form a partnership with you to facilitate your recovery.
  • You will meet with your resource coordinator at your home, at PATH or in the community.
  • You and your resource coordinator will identify goals that you would like to achieve and develop a plan to achieve your goals.
  • You will receive help from your resource coordinator to obtain mental health, substance abuse or other medical treatment.
  • You will receive help from your resource coordinator to obtain services such as housing, health insurance, financial benefits and other needed resources.
  • You will receive assistance from your resource coordinator as you resolve problems and cope with stressors.
  • You will receive support and encouragement from your resource coordinator as you progress in your recovery and become more independent.

Where is the service located?
  • Services are provided in the community, in your home or wherever assistance and support is required.

When is the service available?

Monday through Friday - 8 AM to 6 PM


You are eligible to receive services if:
  • You reside in Philadelphia and you speak English or Russian as a primary language.
  • You have had a psychiatric or psychological evaluation within the past six months recommending Resource Coordination. The person or agency completing the evaluation and referral must submit the packet to the Philadelphia Department of Behavioral Health. We can help you arrange for this evaluation and referral.
  • The Philadelphia Department of Behavioral Health has approved this service for you.
